BLOOD COLLECTION PROCEDURE & X-RAY PROTOCOL


For Blood Collection:
Check the materials enclosed immediately.
You should have one numbered baggie with call name (for each dog).
Inside the baggie you will find:
two bubble wrapped glass lavender top tubes with the dog's blood number on them.
two sealed Vacutainer needles.
two plastic holders for the needles.

Have the dog's blood drawn on Monday, Tuesday, Wednesday, or Thursday.
When the blood is drawn, re-wrap the lavender top tubes in the bubble wrap, secure them with the tie, and put them back in the baggie.   DON'T SEND ANYTHING ELSE BACK!
 

Blood Shipping Instructions:
You have received one FEDEX box, ready to assemble as well as one FEDEX Airbill, already addressed in a clear envelope.

Assemble the box; put the blood in its baggie inside, along with some crumpled paper so it won't rattle around.  This blood is never to be frozen.  Do not include ice packs or dry ice.  Ship the blood the same day it is collected.  Ship only on Monday, Tuesday, Wednesday, or Thursday FEDERAL EXPRESS Standard Overnight delivery.  If you are unable to ship until the next day, put the blood sample in the refrigerator.  It will be okay for one or two days ONLY.

For X-ray Series:
You should have one set of X-ray Protocol sheets (4 written pages, 1 sheet of pictures), a Body Weight collection sheet, and 5 sticky labels with your dog's blood number (one label to be put on each x-ray view) to give to your veterinarian.

Be sure you take these materials to your vet when you have your x-ray appointment.  It is also a good idea to take a quarter (25 cent piece) with you as it is REQUIRED for scale on all x-rays.

When the x-rays are completed, ask your veterinarian to send them WITH HIS BILL, to the address given in the protocol.  Please make sure the name, address, phone number and tax ID of the veterinarian is clearly indicated on the bill.  The veterinarian will be reimbursed directly up to a maximum of $175.00, in a timely manner, when acceptable X-rays are received at the University of Utah.  If your veterinarian has any questions, he or she can call Dr. Lark: 801 581-7364.
 

Is Something Missing?
If either the blood kit or x-ray protocol is not in this package you may already have completed the procedure for this dog.  If this is incorrect, if anything is missing or broken, or if items are marked incorrectly, please call:
